Responsibilities

- Oversees the day to day activities in the home dialysis programs
- Ensures comprehensive, quality care for the patients receiving dialysis in the hospital setting
- Participates in the administration of program policies and procedures.

- Patient Care (shares responsibility with the other home-training nurses)
- Evaluates patients for feasibility of doing hemodialysis/CAPD within their home setting. Provides care before patients start training – drug changes, catheter care, IV or IM medications, fistula/graft care.
- Teaches patients and their partners about dialysis, the machine and the treatment of complications so they can successfully run hemodialysis or CAPD at home.
- Keeps accurate records of the patients' progress while training and while at home.
- Evaluates the patients' progress and well being when they come for clinic check-ups once per month and is aware of any problems or potential problems – physically or with equipment.
- Oversees the ordering of supplies of patents on home dialysis. Provides supplies on emergency basis when needed.
- Continually tries to evaluate equipment and techniques that will reduce home dialysis cost without interfering with the quality of care.
- Provides home visits annually for home hemodialysis patients and as needed for CAPD patients. Makes hospital visits as needed to provide care and/or supplies.
- Assess problems of patients and/or give direction over the phone when needed. Adjust the schedules as needed.
- Initiating treatment according to CAPD protocol.
- Give IV and IM medications
- Obtain specimens and send to the appropriate laboratory.

- Make appointments at other health care facilities when needed by patient.

- Coordination
- Teaches other personnel how to train patients and their partners to run hemodialysis either at home or in a center setting.
- Advises other personnel on alternate approaches to training patients and their partners.
- Takes the leadership in developing audio-visual aides useful in teaching lay persons the techniques of hemodialysis and CAPD. Provides in-service to other healthcare facilities for patient care continuity.

- Unit Responsibilities
- Participates in the orientation and on-going in-service of the hemodialysis staff in cooperation with staff development coordinator.
- Acts as a senior member of the staff in advising newer staff members on how to handle various situations.
- Acts as a stimulus in helping the unit to implement new ideas and programs.
- Helps to staff the unit in times of need.

- Other duties as assigned
- Organizes, arranges and prepares for patient support group meetings, on a regular basis.

Qualifications

- A licensed, registered nurse in the State of Michigan

- at least one year's experience in a home-training program for hemodialysis and/or CAPD/CCPD.

- Two years supervisory experience preferred, preferably in hemodialysis
