Responsibilities
Under minimal supervision, facilitates the care of patients across the continuum of care to ensure quality outcomes and appropriate utilization of health care resources. The case manager works in collaboration with a multi-disciplinary team in the planning, implementation and evaluation of medical and nursing care. The case manager provides education and consultation regarding resources, develops a discharge plan and facilitates and monitors its implementation. Assist patients/families/caregivers to adjust and cope with illness, diagnosis, treatment options and prognosis. Through continuous assessment, screens the appropriateness of services, the utilization of healthcare resources and the quality of patient care in accordance with federal, state, third party payer requirements and accreditation/licensure standards. Performs all job duties and responsibilities in a courteous and customer-focused manner.
EDUCATION AND EXPERIENCE: Bachelors degree in Nursing preferred. Active State of Michigan RN license, or Masters in Social Work (MSW) with State of Michigan certification in Social Work (CSW. ACSW preferred), minimum of three to five years of clinical experience required. (October 2004)
