- Henry Ford Wyandotte Hospital was established in 1926. This 401- bed acute care hospital serves the Downriver region and surrounding communities of southeast Michigan.
- The main Wyandotte campus features a surgery department with 11 operating suites, two endoscopic suites and a robotic surgical system: a 12 bed ICU and 35 bed IMC Unit: a 25 bed birthing center , an emergency department with pediatric specialty care: general medicine and surgical care, ancillary testing, inpatient physical medicine and rehabilitation services: adult mental health services: neurosurgical services: and physician offices.

Responsibilities

Under the direct supervision of the Administrator, Rehab and Orthopedic Service Line, performs proactive case finding internally at HFWH and screens patients for appropriateness for inpatient rehabilitation. Markets / educates referral sources (case managers, therapist and physicians) on inpatient rehabilitation in order to grow our business

Qualifications

MINIMUM AND PREFERRED EDUCATION

- Licensed as an RN in the State of Michigan

- BNS preferred

- CRRN preferred

- Current state of MI registered nurse license required
- Certification as a rehab registered nurse strongly preferred

PREFERRED EXPERIENCE

- Case management experience

- Experience in the rehabilitation of chronically/catastrophically ill or injured individuals is highly preferred.

- Understand regulations and parameters of third party reimbursement necessary.

- Demonstrate excellent skills in oral and written communication, decision making, accountability, analysis, and time management.

- Participate in continuing education appropriate to rehabilitation case management.

- Mental ability to make judgments on appropriate placement of patients based on available clerical data.
